41. Why should pentazocine (Talwin) not be used in treating cancer pain?
The use of agonist/antagonists also should be avoided because of poor oral efficacy and because of the particularly disturbing nature of the psychotomimetic side effects in patients who are already fearful and anxious. Furthermore, pentazocine is a “ceiling” drug; that is, doses cannot be greatly increased to treat increasing pain without greatly increasing the incidence or severity of side effects.
